What is eyelid malposition?

Eyelid malposition, as the name suggests, is an abnormal positioning in which the upper or lower eyelid loses contact with the eye, causing functional issues that can seriously affect the eye.

It can be of three types: ptosis, entropion, and ectropion.

  • Entropion. In entropion, the eyelid margin turns inward so that the eyelashes rub against the eye, causing discomfort that can become severe. There is also a high risk of corneal wounds and infections that may affect vision.
  • Ectropion. In ectropion, the eyelid margin turns outward, exposing the conjunctiva (the inner surface of the eyelid that touches the eye), which becomes dry and irritated, causing constant discomfort. In addition, the lacrimal point shifts outward, preventing it from collecting tears from the tear film, resulting in continuous tearing, which can be very bothersome.

Mild eyelid malposition can progress and cause more serious damage that may affect the eye and vision. That’s why early detection and prompt treatment allow for a timely intervention to stop its progression.

Why does eyelid malposition occur?

Eyelid malposition can have several causes, the most common being aging. But it is not the only one.

  • Aging. The main cause of eyelid deformities and malpositions, affecting facial aesthetics and creating an aged, tired appearance, potentially impairing vision and diminishing quality of life.
  • Trauma, scarring, and surgeries that damage the muscles or nerves responsible for eyelid movement. Whether you notice a severe or mild malposition, it is advisable to consult with specialists for testing and evaluation. Keep in mind that early surgery is crucial to prevent worsening or eye damage.
  • Facial nerve paralysis or Bell’s palsy.

How to detect eyelid malposition early

In cases of entropion and ectropion due to aging, starting at around age 60–65, you may want to attend periodic check-ups to detect the condition early and benefit from timely surgery.

If you notice changes in the appearance of your eyelids, it is recommended to schedule a consultation to determine the cause—whether it’s some form of eyelid malposition—and if so, to learn about the appropriate treatment.

If you experience symptoms compatible with eyelid malposition, you should come in for a consultation and diagnostic testing. Possible symptoms include:

  • Eye redness
  • Gritty sensation or feeling of a foreign body in the eye
  • Corneal irritation or burning
  • In some cases, discharge from the tear duct and/or crusting on the eyelid

What is the treatment?

Treatment for eyelid malpositions, such as entropion and ectropion, is surgical in nearly all cases. This surgery must be performed by Oculoplastic Surgery specialists.

It is an outpatient procedure performed under local anesthesia, lasting approximately 30 minutes, and involves anchoring and tightening the eyelids to restore them to their proper position.

There are two possible surgical approaches for treating entropion and ectropion. The choice of technique depends on the tests performed during consultation, and the specialist selects the best method to ensure optimal results and maximum safety.

  • External approach: the surgery is performed through the skin.
  • Internal approach: the procedure is done through the conjunctiva.

It is rare but possible. However, it does not occur in the same eye simultaneously. You may have ectropion in one eye and entropion in the other. This is because in ectropion, the eyelid folds outward abnormally, and in entropion, it folds inward.
